Job Description

Status: PRN, Non-Exempt

Schedule: 12 Hour variable shifts/hours as needed.

Summary: The Behavioral Health Nurse will provide and coordinate comprehensive care of acutely ill patients of all age groups by assessing patients medical and mental condition, prioritizing patients needs and resource requirements in crisis situations. S/he will plan patient treatment objectives, create time frames, and contribute to crisis intervention. Along with the RN staff as a whole, they will manage violent or potentially violent patients by assessing situations and taking appropriate precautions.

The Behavioral Nurse will partner weekly with the Psychologists, Therapists, Social Worker and the patient/patient families as well as other RN staff members as necessary to discuss the recommended medical treatment per individual. Not only will the RN be creating and implementing medical treatment, they will also be responsible for patient program development on the unit and community involvement/education.

Essential Duties and Responsibilities:
Identifies when resources are needed and properly consults with others in dealing with crisis situations
Educates patients about proper use and effects of medication on an individual or group basis
Maintain the delivery of quality patient care with a specialty in the area of behavioral/mental health
Partner with Psychologists, Therapist and RN staff regarding patient care
Perform patient assessments, develop patients care plans, evaluate patients care plans effectiveness, modify as appropriate, and document all patient care as appropriate
Assesses patients health status through interview, observation and physical assessment skills
Develops an individualized plan of care with specific goals and target date in collaboration with the patient, family, provider and other members of the health care as appropriate
Implements appropriate interventions identified in the plan of care to move the patient toward established goals or outcomes
Systematically evaluates and documents patients progress to expected outcomes / goals on an ongoing basis throughout the shift and modifies plan of care based on patients responses to expected outcomes
Completes documentation in the patient medical record accurately and in a timely manner
Demonstrated technical knowledge and skill to safely utilize patient care equipment.
Prioritizes work based on changes in the patient condition, changes in assignments and unanticipated interruptions
Collaborates with the health care team to organize tasks and activities from the patients admission through discharge
Performs all nursing care activities using techniques to promote safety, reduce risks, and follows policies and procedures
Acts as an advocate for the patient with families, physicians and other healthcare team members
Adapts teaching strategies passed on the patients response, readiness to learn and level of comprehension
Implements cultural, age-specific gender and developmental factors into patient/family education about health concerns, disease specific conditions, self-care and long term management of chronic conditions
Promptly follow up on patients/ family requests/concerns
Provides care to all age groups as defined by nursing standards on each unit
Uses age appropriate communication techniques
Perform tactics to support great patient experience and lead others to support initiatives in patient experience

Required Qualifications:
Diploma or Associates Degree in Nursing
Licensed in the State of Illinois
Advanced skills in reading, writing and oral communication
Demonstrates knowledge of basic skills of professional nurse; the use of clinical information systems, basic skills in keyboarding and use of computer systems.
Certified in CPR or completes certification within 90 days of employment

Preferred Qualifications:
2-3 years experience
